PULLMAN FIRE DEPARTMENT AMBULANCE TRANSPORT MEDICAL RECORDS
Patients who are treated and transported by Pullman Fire Department Emergency Medical Services personnel, may request a copy of their ambulance transport record also known as a Pre-Hospital Patient Care Report and other related documents.
Pre-Hospital Patient Care Reports are medical records, and as such, are protected under Federal and Washington State laws. Pullman Fire Department personnel follow strict guidelines to ensure confidentiality of the patient’s protected health information (also known as PHI). As healthcare providers, we are allowed to use and disclose patient health information for specific purposes without your authorization or notification. If you prefer a paper copy, one will be mailed to you upon request.
To obtain records, the patient or parent/legal guardian of a minor-age patient, must complete and submit the Patient Consent for Disclosure and Release Affidavit of Pre-Hospital Medical Records request form, stating the specific medical record requested, and to whom and where the record should be released to.
